Insurance Coverage
Start your program by focusing on one group of people you want to monitor and develop a treatment plan for. You may want to choose patients with Medicare as a primary insurer, Medicare as a secondary insurer or commercial health insurance covering RPM.
Think about the patients you treat and then choose which patients to target initially with RPM.

FACTORS TO CONSIDER
The Right Circumstances
Other focuses for your program may be patients with two or more chronic conditions, patients with unreliable transportation or patients living far from your practice.

CONDITIONS TO TREAT
Diabetes
Diabetic patients can check their blood glucose levels with glucometers. If a reading is abnormally low or high, the clinical staff can be informed and help the patient get their blood sugar under control. They can also notice trends and can then treat patients to prevent poor outcomes.

RPM can help prevent diabetes, motivating prediabetic patients to take their readings. Specifically, it allows patients to use weight scales so they and their providers can track their weight. This can help patients increase their physical activity and decrease their weight.
Prediabetes
Hypertension is a prevalent issue in the U.S. and is a major risk factor for heart disease and other medical conditions. Patients can use remote blood pressure monitors, and providers can then view their blood pressure over time, noticing trends and helping them get their blood pressure under control.
Hypertension
Obesity can lead to stroke, heart disease, type 2 diabetes and more. Just as with prediabetics, obese patients can use connected weight scales to check their weight and view trends. This helps them comply with daily weigh-ins. Providers can also reach out to patients if they’re not approaching their weight goals.
Obesity
RPM can help patients with COPD manage their symptoms. They can use pulse oximeter devices to measure their oxygen saturation, so they can better understand their disease and receive the appropriate support from providers.
COPD
Patients with chronic kidney disease can use blood pressure monitoring devices, glucometers and connected weight scales together to improve their health outcomes and prevent chronic conditions from progressing.
Chronic Kidney Disease
Patients with heart failure can see if there is an increase in fluid buildup by using RPM weight scales. They can keep comfortable at home, while providers can have the information needed to know if medication adjustments are needed.
